Invitation for Proposals – Caregiver Support Model & Psycho-Education Program on Empowerment: Development and Validation Project
Simon K. Y. Lee Elderly Fund ("The Elderly Fund") is partnering with City University of Hong Kong (“CityU”) to implement the Caregiver Support Model & Psycho-Education Program on Empowerment: Development and Validation Project (“the Project”), a 2.5-year project that aims to provide social workers with a scientific, comprehensive and validated working model and all that are necessary, such as Caregiver Needs Assessment tool (“CNA”), personalized Caregiver Intervention Plan Guidelines, step-by-step Case Monitoring Template, as well as information on caregiving resources and services to support and empower family caregivers of frail elders. The Project is led by Professor Alice Chong, Dr. Marcus Chiu, Dr. Dannii Yeung, and Dr. Ben Li of CityU.
The Elderly Fund is inviting non-governmental organizations(“NGOs”) to be the collaborating NGOs to participate in the Project for 1.5 years. The overall budget range is between $950,000 and $1,150,000. Please click Invitation for Proposals & Application Form for detailed information.
